Medicare Annual Wellness Visits

Medicare Annual Wellness Visits are yearly visits for Medicare beneficiaries to get a health risk assessment (HRA) and create or update a personalized plan that addresses gaps in care, targets disease prevention and promotes a holistic health experience.

If you have had Medicare or a Medicare Advantage plan for longer than 12 months, then you are eligible for a Medicare Annual Wellness Visit at no cost to you. Medicare Annual Wellness Visits are fully covered by Medicare and are eligible to repeat every 12 months.

Medicare Annual Wellness Visits available through The Little Clinic are fully covered by Medicare, meaning they are available to Medicare beneficiaries at no additional cost (eligible for repeat every 12 months).

Coinsurance and deductibles may apply to services beyond the base Medicare Annual Wellness Visit appointment. Visit Medicare.gov to learn more.

Medicare Annual Wellness Visits and Annual Physical Exams are commonly mistaken as the same thing. However, they’re entirely different types of visits. Here are the major differences:

Annual Physical Exam

• Copayment applies.
• Blood work and lab tests are completed.
• Addresses new health problems.

Medicare Annual Wellness Visit

• Fully covered by Medicare once every 12 months.
• Creates a 5-10 year personalized prevention plan to keep you on track.
• Provides guidance specific to your current and future health needs.

While Annual Physical Exams are designed to check a patient’s overall health, Medicare Annual Wellness Visits go one step further to ensure that you and your provider are aligned on preventative health goals.

After completion of a health risk assessment (HRA), our healthcare providers will identify gaps in care and assist in developing a plan to address those gaps.

Several gaps in care can be addressed at our Clinic by one of our healthcare providers during the same or a subsequent visit. Examples include vaccinations, colon cancer screenings, tobacco cessation counseling and diabetes care.

Other gaps in care may be referred to providers outside of our Clinic. Examples include eye exams, audiology tests, mammograms and nutrition consultations.

We can also collaborate with your primary care provider (PCP) to deliver appropriate follow-up care.
